Calibration and Dilation Are Not the Same Procedure
Calibrating a urethra and dilating one are different procedures, and the instruments are not interchangeable. A bougie à boule carries a bulbous head on a slender shaft: it is passed to find the point at which the lumen narrows, and because the bulb measures rather than stretches, properly performed calibration does not dilate the urethra and carries correspondingly little risk. That distinction matters clinically, because a stricture should be assessed before it is treated.
A sound does the opposite. It is solid, tapered and passed in ascending sizes to stretch the lumen open, and each pass is a therapeutic act rather than a measurement. French Sizing, and What to Do When Nothing Passes
Size in urology is expressed in French, and the conversion is fixed: one French is one third of a millimetre of outer diameter, so 8 Fr is 2.67mm, 24 Fr is 8.00mm and 30 Fr is 10.00mm. A normal penile urethra calibrates at around 30 Fr, with the glans and fossa navicularis narrower at under 25 Fr, and dilation proceeds in 2 Fr steps rather than jumps.
Where a stricture is too tight for any sound to pass, the filiform and follower system takes over. A fine flexible filiform is negotiated through the narrowing first and left in place as a guide, and progressively larger followers are then threaded over it, so the dilator is always tracking a known path rather than searching for one. Prostatectomy, Renal and Bladder Instruments
Beyond the urethra the range covers open urological work. Prostatectomy needs both traction and exposure: Lowsley Prostatic Tractors position the prostate from within the urethra, and Young Anterior, Bulb, Bifid and Lateral Retractors hold the perineal exposure open around it. Prostatectomy clamps control the capsular bed once the gland is enucleated.
Renal and bladder work has its own instruments. Material, Reprocessing and Certification
Every pattern is forged from German stainless steel and passivated for corrosion resistance across repeated cycles. On instruments that enter the urethra the inspection point is the entire working surface, not just the tip: run a gloved finger along the full length of every sound and bougie and confirm it is smooth and unmarked, because urethral mucosa is thin and a burr anywhere on the shaft will score it on withdrawal as readily as on insertion. Sight down each shaft to confirm the curve is true, since a bent sound no longer follows the anatomy it was shaped for, and confirm size markings are still legible. Clean immediately with a neutral-pH enzymatic solution, flushing every cannula and lumen, then rinse with deionised water and dry fully before autoclaving at 134°C.
