Why Jaw Design Decides How Much Treatment Is Possible
An expressor works by trapping the eyelid between two surfaces, and the surface behind the lid does two jobs at once. It shields the globe from the force being applied through the lid, and it gives that force something solid to work against, so the pressure goes into the gland instead of into the eye.
That is also where the pain comes from. Clearing an obstructed duct needs real force, and the amount of treatment a patient will accept in one sitting is set by how that force feels, not by how many glands need clearing. Jaw geometry is the lever: broad, smoothly polished paddles spread the same load across more tissue, so pressure at any one point stays lower for the same effect on the gland. Paddle Width and Approach Angle
Paddle width and approach angle separate the forceps patterns. Collins Expressor Forceps (PS-J-145, $31.87) and Ophthalmic Collins Expressor Forceps (PS-J-116, $25.26) carry the Collins configuration, a slightly angled 95mm instrument with 8.0mm wide paddle jaws and a flat handle, built for firm expression along the lower lid margin. Tearse Meibum Expressor Forceps (PS-J-144, $27.86) uses a different angle set so the tips can be positioned parallel to the lid margin from either a temporal or nasal approach, and it works on upper and lower lids equally.

How to Use a Meibomian Gland Expressor
Technique decides the result more than the instrument does, and the sequence is fixed. Heat first: meibum has to reach melting temperature before it will move, and a warm compress alone does not get there. Express within a few minutes of heating, because the glands cool again in under a minute once the heat source is removed.
Then work slowly. Pressure at one point should be held for around thirty seconds rather than swept along the lid, and all four lids should be treated. The upper lids matter more than the time usually given to them, since they produce roughly sixty percent of the meibum and are the ones most often rushed. Anaesthetise first, work at the slit lamp, and reposition the patient rather than the instrument when the upper lid is difficult to reach.
Material, Reprocessing and Certification
Every pattern is forged from German stainless steel and passivated for corrosion resistance across repeated cycles. The inspection point is the paddle face, and it matters more here than on most instruments: run a gloved finger across both faces and check for burrs, pitting or loss of polish, because that surface presses directly against tarsal conjunctiva and a rough face turns a tolerable procedure into one the patient stops. Confirm the jaws close parallel with no lateral offset, since a paddle meeting at an angle concentrates pressure on one edge. Clean immediately with a neutral-pH enzymatic solution, then rinse with deionised water and dry fully before autoclaving at 134°C.
