Why a Depressor and Not a Retractor?
A depressor works by displacement rather than retention: the blade is slid along the vaginal wall and held there by the examiner's hand, so it can be repositioned continuously as the field changes. That is the opposite of a self-retaining retractor, and it is the point of the instrument. Sims' speculum retracts the posterior wall and gives wider exposure than a Cusco pattern, but because it does not hold itself, the examiner reaches for a depressor to clear the anterior wall and complete the view of the cervix.

From Malleable Copper to Fixed Steel
The pattern goes back to J. Marion Sims, who described it in Uterine Surgery in 1858 as a way of drawing the cervix forward by pressing in the anterior cul-de-sac, and named it the uterine depressor. His original was malleable copper on an ebony handle, bendable by the operator to suit the case in front of him.
Stainless steel replaced that, and the trade is deliberate. A modern depressor holds one fixed curve instead of being reshaped between patients, which means the blade profile is consistent, the finish can be polished smooth enough to clean properly, and the instrument survives repeated autoclaving without work-hardening at the bend. What was adjustability in 1858 is now reproducibility.
Material, Reprocessing and Certification
The depressor is forged from German stainless steel and passivated for corrosion resistance across repeated cycles. The inspection point on a single-piece instrument is the blade edge and the finish: run a gloved finger along both edges to check for burrs or nicks that would catch mucosa, and check the polished surface for pitting, since a dulled or pitted finish holds soil that cleaning will not lift. Clean immediately after use with a neutral-pH enzymatic solution, then rinse with deionised water and dry fully before autoclaving at 134°C.
