Why a Screw and Not a Grasping Jaw
A fibroid sits inside a pseudocapsule, and enucleation works by finding that cleavage plane and opening it bluntly rather than cutting the tumour out. Doing so needs counter-traction: something has to hold the fibroid firmly enough to be levered against the uterine wall while a probe works the plane. A grasping jaw cannot do it, because a fibroid is dense, smooth and heavy, and jaws slip off it under exactly the load that matters.
A threaded tip solves that by engaging inside the tissue rather than around it. The screw is turned into the myoma until it seats, and from that point the traction is axial and does not depend on grip pressure. 152mm: Open, Laparoscopic and Morcellation
At 152mm the screw sits at the standard working length, long enough to reach a fibroid seated deep in the uterine wall and short enough to stay controllable in an open field. The same length makes it usable through a laparoscopic port, where reach has to clear the abdominal wall before the tip meets anything.
Traction is not the only job it does. During morcellation, the screw holds the fragment while it is cut, keeping a heavy, mobile mass under control. In open myomectomy, it earns its place where fibroids are large or multiple, because those are the cases where the uterus has to be repositioned repeatedly and a hand alone cannot hold the tumour steady through the dissection.
Material, Reprocessing and Certification
The screw is forged from German stainless steel and passivated for corrosion resistance across repeated cycles. The inspection point is the thread and the tip: check the point under light for blunting, because a dulled tip has to be forced rather than turned into the tissue, and run a gloved finger along the thread to confirm the flights are unflattened. Check the shaft is true, since a bent shaft transmits traction off-axis. Clean immediately with a neutral-pH enzymatic solution, brushing along the thread flights where tissue lodges, then rinse with deionised water and dry fully before autoclaving at 134°C.
